Adult Foster Care
The Adult Foster Care (AFC) program is for individuals that is 16 years and older with a developmental, intellectual, physical or mental disability who cannot safely live alone. The member caregiver is usually a friend or a family relative that already live with the individual that require the help. We train that friend or family to become the individual caregiver. As the caregiver, they are able to continue to provide daily care to their love ones. The program was design for that needs help daily with personal care, but want to live in a family setting rather than in a facility. The caregiver providers meals, companionship personal care assistance and 24 hour supervision. In the past year, the AFC program has increased its numbers. A full time Case Manager and RN for the program. There is a monthly visit to the family and caregiver once a month by the RN and case manager.
– Have MassHealth Standard
– Be at least 16 years old (legal guardian or spouse cannot be the caregiver) and older
– Services must be ordered by a physician
– Require supervision, cueing or physical assistance with one or more activities of daily living; mobility, eating,
dressing, bathing, toileting, or transferring.
